How do I make an appointment?
Please call the Family Health Center most convenient to you. We will be happy to schedule an appointment and give you any additional information at that time.
What are your office hours?
By appointment only from 9am-5pm, Monday through Friday.
What should I bring with me to my first appointment?
- Photo ID
- Health Insurance Card(s) including Pharmacy card
- List of all medications you’re currently taking, along with dosages
- Medical records from previous Physicians
- Please plan to arrive 15 minutes early to complete paperwork
What do I need to do if I have a medical problem after office hours?
For life-threatening emergencies please dial 911 for assistance. For urgent medical problems that occur after office hours and cannot wait until the next working day, please call Winter Haven Hospital at (863) 293-1121 and ask the operator to page the physician on call for the Family Health Centers.
Do I require a Referral to see a Specialist?
With most health insurance plans, you will probably require a referral from your PCP before a Specialist will see you. Your PCP will assist you in determining if a referral to a Specialist is needed. If it is decided that you need to see a Specialist, our office will make that appointment for you.
How do I obtain a prescription refill?
For prescriptions written by a Family Health Center Physician, please call our office at least two weeks before your prescription runs out. Please have the name of the medication, the prescription number from the bottle, and the pharmacy phone number.
What Health Insurance Plans do you participate with?
We accept Medicare and are contracted with most medical insurance plans including many commercial HMO’s and PPO’s. Please call a Family Health Center to determine if we participate with your insurance plan.
Will I have to make a payment when I come in for my appointment?
If your health insurance plan requires a co-payment, we will collect it when you come in for your appointment, before you see the physician. We will be happy to file your insurance claims with participating insurers on your behalf.
What methods of payment do you accept?
We accept cash, check and most major credit cards.
