Our online portals make it easy to get your hospital, medical group and urgent care medical records. You can submit a request from your smartphone, tablet or computer, and your records will be delivered right to you via email or by mail.

You can also download our printable Release of Information Form and mail to the hospital where you received services. Please note: We must have your signature to release the records you request. To protect your confidentiality, all patients 18 years of age or older must sign the Release of Information Form. A parent or a legal guardian may sign for children under the age of 18.

To request medical records from any of BayCare's imaging centers, please visit BayCare Outpatient Imaging.

Your lab results will be sent to your physician within two business days, with the exception of some special tests, which may take longer. For your convenience, our staff can tell you if your results may take more than two business days for your physician to receive. Also, if you have an out-of-town or out-of-state physician, please give us the physician’s contact information, and we will forward your test results. For a copy of your results, you may contact your physician’s office directly.

- Medical Power of Attorney (must state medical information in POA)

No, certain records must be requested from their respective departments. Examples of records that must be obtained from departments include radiology, nuclear medicine and cardiology films.

We keep your medical records for 10 years.

We typical take under 5 days to fulfill your medical records request. Additional time may be required for certain requests. We can deliver records to your personal email or upload to your BayCare portal which can reduce the fulfillment time significantly. We also will call you when your records are available should you want to pick them up instead of having them mailed.

Yes, we will be happy to fax or mail your records at no charge to your doctor’s office or treatment center. Please indicate this information on the Release of Information Request form.

Charges for paper copies are per page, postage and sales tax may apply. We do NOT accept CASH. There is no charge when records are delivered electronically direct to your personal email address.
