BayCare wants the best care for you and your family. We know that scheduling regular doctors’ appointments or having to go to urgent care or the hospital can be difficult. For patients who need regular monitoring, BayCare offers @Home, a program allowing patient vitals to be checked from the convenience of their own home.

How It Works

With @Home, physicians and nurses can check on patients in their homes, using tablets and smart medical equipment. Patients enrolled in @Home submit daily readings of their blood pressure, pulse and weight to our experienced central monitor Registered Nurses. If one of their readings produces an alert in the system, a follow-up video session is scheduled  immediately from the patient's home with a nurse. These video visits allow a nurse to see the patients’ body language, skin appearance and facial expressions. This helps us to get a better understanding of the patient’s clinical situation.

While our patients are enrolled in @Home, family members (local or far away) can participate in video calls with their loved one. The patient’s care provider may also be engaged in the video conference. This video technology allows everyone to hear the patient’s care plan and have their questions answered.

The program monitoring of the patient via tablet and daily collection of vitals provides physicians and nurses information to spot any clinical issues that could arise and intervene before the patient’s health declines.
