Comprehensive Care for Joint Replacement Model

BayCare is participating in a Medicare initiative called the Comprehensive Care for Joint Replacement (CJR) model. The CJR model aims to promote quality and financial accountability for care surrounding lower-extremity joint replacement (LEJR) procedures, typically referred to as hip and knee replacements.

BayCare is working closely with physicians and other health care providers to help patients experience more efficient, high-quality care from the inpatient stay through recovery. The CJR model will not restrict your access to care for your medical condition or your freedom to choose your health care providers and services.

BayCare has entered into financial arrangements with collaborating physicians who are engaged in care redesign, and who may furnish healthcare services to you. Under these agreements, BayCare may share payments received from Medicare as a result of reduced spending with collaborating physicians. Collaborating physicians must agree to participate in the quality improvement activities associated with the CJR program.
