Skilled Nursing

Wound Care

Our nurses are specially trained to manage wounds and prevent infection during your healing process. For more complex wounds, we have a team of nurses who are certified in using evidence-based protocols from the Wound, Ostomy and Continence Nurses Society. We have a robust virtual wound care program that uses the most current technologies to monitor wound progression and make sure that you’re healing properly.

Infusion Therapy

Infusion therapy refers to medications or fluids that are given intravenously. Your home care nurse will administer the medication, educate you on any potential side effects and teach and train you and/or your caregiver on how to administer the physician-ordered intravenous medication or fluids.

Disease Management

BayCare HomeCare offers disease management programs for patients who have specific chronic health conditions. Using evidence-based clinical pathways and educational tools, we’re able to identify problems, report changes to your physician, and provide immediate education and support to help improve your health and help you avoid emergency room visits and hospitalizations.

Medication Management

Your BayCare HomeCare nurse will help make sure that you’re taking the right medications in the right dosage, at the right time, according to your physician’s orders. We’ll educate you and/or your caregiver on proper dosage, frequency and potential side effects. We partner with your physician to make sure medications are working and controlling your symptoms.
