Primary Care/Behavioral Health Integration

BayCare Behavioral Health has developed a cutting-edge program for patients of participating primary care offices in Pasco and Pinellas counties. The Primary Care/Behavioral Health Integration Program includes on-site counseling services in a primary care setting. This provides a convenient, patient-centered, holistic approach to care that addresses physical, mental, and developmental health and promotes total wellness. Primary care physicians in participating offices must make referrals into the program for patients to qualify.

 
Licensed counselors are available to evaluate and treat adults and families struggling with a variety of issues including:
  • Mood/affective disorders
  • Suicide ideation/crisis
  • Sexual, physical and/or emotional abuse
  • Trauma
  • Anger management
  • Psychotic disorders
  • Eating disorders
  • Substance abuse
  • Anxiety/excessive worry

  • Grief and loss

Accessing the Integration Program

Primary care physicians at participating offices must make referrals into the program for qualified patients. If you are a patient at one of the above offices and would like to participate, please contact your doctor. Physicians can call (727) 849-4063 to make a referral. BayCare is proud to offer this unique service to individuals throughout Pasco and Pinellas counties in New Port Richey, Largo, Clearwater, Palm Harbor and St. Petersburg.
