Outpatient Medical Rehabilitation

Winter Haven Hospital Outpatient Medical Rehabilitation provides Physical Therapy, Occupational Therapy, Speech and Language Therapy, and Neuropsychological Services for persons with limiting conditions due to a medical condition. We treat persons diagnosed with but not limited to:

  • Disorders of the bones, muscles or joints
  • Back, shoulder, and knee injuries
  • Sports injuries
  • Amputations
  • Arthritis
  • Pelvic floor weakness
  • Spinal cord injuries (based on level of injury)
  • Gait disturbances, Vertigo
  • Brain injury
  • Dementia, Memory Loss
  • Parkinson’s disorder
  • Swallowing disorders
  • Voice disorders

Our clinic is accessible to the public and is open Monday through Friday from 8:00 a.m. to 5:00 p.m. Our patients are referred by physicians for outpatient therapy. We have extended hours upon request.

The frequency and duration of your therapy is dependent on physician orders, progress of treatment and authorization from your funding source. Provision of service may be affected by your payor source requirements. 

Winter Haven Hospital Outpatient Rehab accepts commercial insurance, Worker’s Compensation, Medicare, Medicare Advantage plans, Medicaid, and Medicaid MMA plans. We arrange for cash accounts as needed. We are part of a not for profit health system, BayCare Health.

Our patient population is 18 years old through adult with physical, or cognitive impairments that affect the activities you do every day, such as your ability to walk or move, sports activities, work, speak, or stay home alone. (Teenagers ages 13 and older may be seen for orthopedic sports related injuries ONLY, including ACL repairs, fractures or strains and sprains.)

Our therapy team will provide assistance in returning you to your highest level of activity. Our team members are sensitive to cultural and spiritual issues that may arise with a change in a person’s abilities. Referral to a Psychologist or the Center for Behavioral Health is possible for people expressing difficulty with adjusting to their physical problems. An RN/Case manager and a Vocational specialist are available to assist with medication, education, return to work, and connecting with community resources for the patient and family. Transportation plans can be investigated if the patient is having difficulty getting to therapy appointments.

The Medical Outpatient Rehabilitation program reviews outcomes of our past populations to determine a treatment plan for you, based on your status when you enter the program and the outcomes of people with similar difficulties seen in this program. We look at national data for similar diagnoses and use this data to measure our success.

Admission Criteria:
  • Persons served is age 18 or older
  • Diagnosis is appropriate for outpatient services
  • Medically stable and able to actively contribute in an outpatient rehab program
  • Identified funding source
  • Not harmful to self or others
  • Drug and alcohol free for duration of the treatment program

Discharge/Transition Criteria:

(Discharged criteria is established at the initial evaluation)

  • Reached maximum benefit from therapy or have achieved long term goals
  • Noncompliance with their program through lack of attendance (3 cancellations or 2 no shows)
  • Referred back to physician with recommendations
  • Independent in a home program and no longer requires skilled care
  • At a level of functioning which is better served in another program. (Efforts are made to assist with transition to another level of care)
