How many hours of therapy will I need each day?

Patients are expected to participate in one to two hours of therapy each day. Therapy needs are individually tailored based on diagnosis and medical condition. A therapist will evaluate your status, and together you and your therapist will create a therapy plan that includes high priority treatment goals, timelines to track progress, and the type of therapy (physical, occupational, and speech) required to achieve your goals. Therapy plan will be designed to discharge you home at your highest functional level. 

What are the rooms like?

Rooms at Madonna Ptak Morton Plant Rehabilitation Center are designed to enhance each patients’ sense of home by providing a bright and clean environment that ensures quality of life, and overall patient and team member safety. Each room features high ceilings, large windows, individual television with cable TV, individual phones with direct numbers, and private bathrooms. Your room is kept clean by a team of dedicated housekeepers, and no matter the length of your stay we want you to experience a clean comfortable environment. 

Who will be my doctor?

At Madonna Ptak Morton Plant Rehabilitation Center you will be assigned a facility credentialed doctor who will be responsible for overseeing care during your stay. 

Are there any visitor restrictions?

No, however we ask visitors who are sick or experiencing any flu-like symptoms, including fever, cough, and congestion or sneezing, please do not enter in the facility.

The No Patient Left Alone Act

This Act Guarantees Florida families the fundamental right to visit their loved ones who are receiving care in hospitals, hospices, nursing homes, assisted living facilities, and intermediate care facilities for the developmentally disabled (long-term care facilities). No health care facility in Florida may require a vaccine as a condition of visitation. The Madonna Ptak Morton Plant Rehabilitation Center allows in-person visitation in all the following circumstances, unless the resident objects:

  • End-of-life situations
  • A resident, who was living with family before being admitted to the provider’s care is struggling with the change in environment and lack of in-person family support.
  • A resident that is making one or more major medical decisions.
  • A resident that is experiencing emotional distress or grieving the loss of a friend or family member who recently died.
  • A resident that may need cueing or encouragement to eat or drink which was previously provided by a family member or caregiver.
  • A resident, client, or patient who used to talk and interact with others is seldom speaking.

Additionally, the resident has the option to designate a visitor who is a family member, friend, guardian, or other individual as an essential caregiver. Madonna Ptak Morton Plant Rehabilitation Center will allow in-person visitation by the essential caregiver for at least two hours daily in addition to any other visitation authorized by the provider. 

If you or your loved is met with resistance from our team when attempting to visit with your loved ones, you may ask to speak with the Administrator or Nurse manger. You may also file a complaint with the Agency for Health Care Administration (AHCA) for further review and action online at www.ahca.myflorida.com/visitation or call our dedicated phone line for visitation related complaints (888) 775-6055.

What are the visiting hours?

Visitors may visit any time as long as it is not disruptive to other residents. After hours visitors may access the facility by ringing the bell located at the front entrance. 

May I visit a resident who has COVID-19?

Yes, visitors are allowed at all times. Madonna Ptak Morton Plant Rehabilitation Center follows CDC guidelines that reduce the spread of COVID-19 by maintaining social distancing, and appropriate personal protective equipment (PPE) usage. Face masks, face shields, gowns, and gloves are encouraged and available at the Center.  

What about transportation?

Transportation from the hospital to Madonna Ptak Morton Plant Rehabilitation Center will be arranged by the hospital social worker. Family members are encouraged to provide transportation to all outside appointments such as follow-up doctor appointments, chemotherapy, radiation, or dialysis. The facility employs a driver to assist residents to and from service locations and during approved activities. Should you need assistance with transportation, we have social services to help with arrangements with a wheelchair transport company.

What personal items should I bring?

You play a vital role in maintaining safety during your stay with us. Below are tips that will help you and your family partner with us to maintain a secure and safe environment.

You may bring household items (flowers, plants, small photographs) to make a room look beautiful. We encourage the use of plastic containers as glass may break easily.

Use your discretion when bringing expensive or sentimental items. It is strongly encouraged not to bring any highly valued items into the facility. Please do not bring any items that cannot be replaced if broken, lost, or damaged.

We discourage you from keeping cash and credit cards in your room. These items are better left at home or with a family member.

Please do not bring electrical extension cords, surge protectors or other outlet multiplying devices. The facility follows state fire safety rules. Our maintenance team will assist you with your power needs.

Personal items such as favorite soaps, shampoos and lotions that help you feel comfortable are encouraged. Toothbrushes and other mouth care items are available at the facility; however you may bring your own toiletries and hygiene items. 

What clinical services do you provide?

Madonna Ptak Morton Plant Rehabilitation Center employs a full-time Advanced Nurse Practitioner that works in collaboration with your attending doctor to provide a full range of health care services. Our Infection Preventionist offers educational services on proper infection prevention practices and ensures that infections are isolated, and the spread is limited.  Wound care services are provided by a nurse team who are experts in assessing and treating wounds. A registered nurse is available 24/7.

What services do social workers and case managers offer?

Your case manager will assist you with developing a well-planned, coordinated discharge to ensure your successful return home. Case managers will assist you in making arrangements with home health care and equipment needs upon discharge. Our social worker will partner with you during your stay to ensure your psychosocial needs are meet. 

How do I find out billing information?

Business office personnel are available to meet with your or your family to discuss billing procedures, insurance questions and payment requirements.

How is my treatment plan decided?

Your rehabilitation and skilled nursing plan is determined by you, your doctor, your therapist, nurse and case manager. Knowing what goals you want to accomplish while in the rehabilitation setting will be very important. Together, with your input and that of the interdisciplinary team members, a plan of care will be designed to meet your needs. Meeting those goals will be discussed with you throughout your stay. At any time if you or your family member has a question, we are here to answer them. Together we will work toward getting you back to your highest level of function. 

What technology is available?

Madonna Ptak Morton Plant Rehabilitation Center integrates leading technology to support your health and quality of life. Our comprehensive communication system keeps us connected to you and your family through regular updates via phone and email. Electronic signature technology enables simple and convenient document turnaround. The Center’s wireless network supports a wide variety of personal devices and technologies. Two patient dedicated desktop computers are available on either floor of the center. Our activities department is happy to assist you in setting up virtual meetings with family and loved ones. Free translation aids and technologies are available for those in need of communication assistance. 

Are you able to accommodate my meal preferences?

Yes! We offer a wide range of meal options. Our registered dietitian is available to meet with you and review available options that are appropriate for your health needs and preferences.  

What if I have compliments or concerns?

In support of our mission to provide high quality, patient-focused care, your feedback is welcomed and is used to improve satisfaction and to identify areas to improve our care.
Some of the ways to compliment our team or share your concerns include: 

  • Compliment the team member by filling out one of our “Applause” forms located at the nursing station.
  • A concern form can be completed by any member of our team or if you prefer to complete one yourself, they are located on the wall by the main entrance. These concerns are reviewed by the leadership team daily and a member of our team will partner with you to provide timely resolution to your concerns.
