As a 5-star rated facility by The Centers for Medicare and Medicaid Services (CMS), the Madonna Ptak Morton Plant Rehabilitation Center is among the highest quality nursing home facilities in the state. The CMS Quality Five-Star Rating helps patients, their families and caregivers compare the quality and performance of nursing homes more easily. The rating scale is based on a review of three key areas: health inspections, staffing and quality of resident care measures.

This 120-bed center offers physical, occupational, speech and recreational therapy along with skilled nursing, wound care, pain management and many other services. Specialized procedures and programs are available for neurological, cardiac, orthopedic, osteoporosis and arthritis patients as well as those with complex medical conditions.

Patients with neurological conditions, such as Parkinson’s, stroke, epilepsy, multiple sclerosis, and balance disorders benefit from a variety of therapies, including electric stimulation that help patients overcome swallowing difficulties.

Stroke patients also receive the highest level of care available, as evidenced by the Gold Seal of Approval™ for primary stroke care, awarded to Morton Plant Mease from The Joint Commission.

An experienced and highly credentialed medical staff helps patients regain their independence as quickly as possible. Patients retain relationships with their physician from the hospital, who work closely with therapists and nurses to deliver personalized care. A subacute unit takes care of the most seriously ill patients and offers a nurse-to-patient ratio of 1:8.

The center delivers one-on-one and group therapy sessions designed for each individual patient’s needs. Because the rehabilitation center accepts patients around the clock every day of the year, patients can be moved to a more comfortable environment immediately after their discharge, instead of spending a weekend or holiday in the hospital waiting for their transfer.

Madonna Ptak Morton Plant Rehabilitation Center’s patient care team also includes dietitians, case managers and psychiatry services to help patients and their families address both the mental and physical aspects of a disability. Each patient is also served by a social worker, who can help assure access to a variety of area services after discharge. Medical staff members meet regularly, and the entire patient care team discusses treatment plans and objectives with patients, their families and their caregivers.
