Diabetes Education Empowerment Program (DEEP) Virtual

Enrollment Criteria

Please download, and return a completed event consent form to GetHealthy@BayCare.org prior to your first session. Include in your email, the date and title of your event. The event consent form can be located under the content tab by clicking here. Requirements to participate in this class are: Must be 18 years or older, Live and/or work in Pinellas, Pasco, Polk or Hillsborough County. Must have diabetes, pre-diabetes or caretaker status - physician referral not required. Class meets once a week for six weeks. Terms of Participation
