Request Medical Records Online

Our online portals make it easy to get your hospital, medical group and urgent care medical records. You can submit a request from your smartphone, tablet or computer, and your records will be delivered right to you via email or by mail.

Release of Information Form

You can also download our printable Release of Information Form and mail to the hospital where you received services. Please note: We must have your signature to release the records you request. To protect your confidentiality, all patients 18 years of age or older must sign the Release of Information Form. A parent or a legal guardian may sign for children under the age of 18.

Imaging Centers and Laboratories

  • Outpatient Imaging

    To request medical records from any of BayCare's imaging centers, please visit BayCare Outpatient Imaging.

  • Outpatient Laboratories
    Your lab results will be sent to your physician within two business days, with the exception of some special tests, which may take longer. For your convenience, our staff can tell you if your results may take more than two business days for your physician to receive. Also, if you have an out-of-town or out-of-state physician, please give us the physician’s contact information, and we will forward your test results. For a copy of your results, you may contact your physician’s office directly.

    Lab results can also be obtained in the BayCare Patient Portal at MyBayCare.org. Learn more about how to enroll in the Patient Portal.

Medical Record FAQ

  • Who can sign the Release of Information form?

    Any of the following people can sign the Release of Information form:

    • Patient – 18 years of age or older
    • Legal next of kin
    • Emancipated Minor
    • Healthcare Surrogate
    • Medical Power of Attorney (must state medical information in POA)
  • Can I get all my records from the Medical Records Department?
    No, certain records must be requested from their respective departments.  Examples of records that must be obtained from departments include radiology, nuclear medicine and cardiology films.
  • How long does the hospital keep my records?
    We keep your medical records for 10 years.
  • How long will it take you to process my request?
    We generally take 4-5 business days to fulfill your medical records request. Additional time may be required for certain requests. We will call you to come and pick up your records if you do not want the copies mailed.
  • Can you send my medical records to my doctor for me?
    Yes, we will be happy to fax or mail your records at no charge to your doctor’s office or treatment center. Please indicate this information on the Release of Information Request form.
  • Is there a charge for copying my medical records?
    Charges are per page, postage and sales tax may apply. We accept Check, Money Orders, Debit or Credit Cards. We do NOT accept CASH at this time.
  • Can I request a copy of a birth certificate from you?
