Outpatient Medical Rehab Program

Winter Haven Hospital’s Outpatient Rehabilitation Center offers physical therapy, occupational therapy, speech-language pathology and neuropsychological services for persons with sports injuries, musculoskeletal injuries or dysfunction, post-surgical orthopedic limitations, pelvic floor disorders, neurological conditions, cognitive deficits, swallowing and voice dysfunction, spinal cord injuries, emotional/behavioral problems associated with a medical condition, and other diagnoses who could benefit from therapy. A referral/prescription from a Florida-licensed physician is required for treatment to begin.

In addition our team also includes a RN/case manager and a vocational evaluator to assist with medication education, back to work concerns, and resources available in the community.

Our facility is a hospital-based outpatient setting that is accessible to the public and is open Monday through Friday from 8:00am to 5:30pm. We are located on first floor of Winter Haven Hospital near the South Entrance. Persons are referred by physicians or case managers. We are accessible by public transportation. If eligible, our staff can assist in applying for transportation services through the county’s Transportation Disadvantaged Program.

The frequency and duration of treatment is dependent on physician orders, therapist recommendation, and authorization from your funding source. Provision of service may be affected by payor source requirements.

Winter Haven Hospital Outpatient Rehabilitation Center accepts Worker’s Compensation, Brain and Spinal Cord Injury Program and most commercial insurance, Medicare, Medicare HMO and Medicaid programs. The financial counseling department of Winter Haven Hospital can arrange payment plans for cash accounts. The persons we serve are provided an estimate of their fees. Winter Haven Hospital is a not-for-profit hospital.

We treat persons 18 and older with limitations/impairments that affect normal activities, ability to walk or move, ability to think, and to manage daily living. Persons 13 or older with orthopedic injuries including ACL repairs, fractures or strains and sprains are also considered for therapy on a case by case basis.

Our therapy team provides assistance in returning persons to their prior level of activity. The team is sensitive to cultural and spiritual beliefs and values.

When persons reach their goals they are discharged with instructions. If progress is impeded by decline in medical status a transitional plan is discussed with the person and family for transfer to a more appropriate level of care.

The Outpatient Medical program reviews outcomes of past population to predict outcomes for current treatment. We look at national data for similar diagnoses and use this data to measure our success.

Admission Criteria:

  • Age 18 or older
  • Medically stable
  • Prescription from a Florida-licensed physician
  • Diagnosis appropriate to outpatient rehabilitation including, but not limited to, orthopedic injury, musculoskeletal dysfunction, post surgery(orthopedic),spinal cord injury, neurological condition, chronic pain, multi-trauma, cognitive dysfunction and other diagnosis for which therapy provides an expectation of improvement in functional status. The following specialty interventions are also included in Outpatient Medical Rehabilitation:
    • Pelvic floor disorders (male and female)
    • Pre-driving assessments
    • Orthopedic injuries (age 13 or older – considered on a case by case basis)
    • Vestibular therapy
    • Voice and swallowing dysfunction
    • Spinal cord injury, pending review by admissions team. As a rule, who are not dependent upon a ventilator/respirator, are nontraumatic or traumatic, at level C4-5 if incomplete, and if lower, complete or incomplete
    • Neuropsychological evaluations
    • Wheelchair evaluation
  • Referrals reviewed by admissions team
  • Ability to tolerate and participate in outpatient rehab program
  • Reasonable expectation of improvement in function
  • Identified funding source or approval from financial counseling
  • Not harmful to self or others
  • Free of unprescribed drugs and alcohol for duration of the program
  • Outpatient rehab is deemed the most reasonable/appropriate level of care (by physician, family members, patient, and Outpatient rehab team)

Discharge/Transition Criteria:

  • Reached their maximum benefit from therapy or have achieved their long term goals.
  • Non-compliance with their program through lack of attendance (3 cancellations or 2 no shows.) Patient is referred back to their physician with recommendations.
  • Independent in a home program and no longer requires skilled care
  • At a level of functioning which is better served in another program or agency. Efforts are made to assist with transition to another level of care
