Florida Assertive Community Team (FACT)
Program Description
The primary goal of the Harbor's FACT Team is to work in partnership with our local community to provide services and to ensure safety, well-being and self-sufficiency to individuals with psychiatric disabilities who have not been served adequately within the traditional continuum of care. This philosophy is reflective of the identified values of the Department of Children and Families, which include: Recovery, Empowerment, Choice, Cultural Competence, Person-Centered, Stakeholder Inclusion and Rights of the Person Served. We consistently incorporate these values into the FACT service delivery system.
The objectives of the FACT Team are:
- Reduce hospitalizations
- Increase days in the community
- Enhance the quality of life
- Lessen the families’ and significant others’ role of providing care
- Divert possible incarcerations
- Improve social, emotional and vocational functioning/responsibilities
- Promote recovery through the lessening and/or eliminating of debilitating symptoms of mental illness
The Suncoast Region Substance Abuse and Mental Health Program Office will approve all admissions to the FACT Program within 5 working days upon notification by The Harbor FACT Team. The FACT Team will accept for enrollment all individuals referred by the Suncoast Region Program Office and discharged or transferred from other FACT Teams within the state.
Hours:
24 hours per day, 7 days per week.
