Since 1975, BayCare HomeCare has offered local communities throughout Florida traditional home health care, utilizing the highest standards of quality in the industry. As a member of community-owned BayCare Health System, we are a not for profit provider of homecare.
Each Home Health agency offers the expertise of registered nurses (RN's), licensed practical nurses (LPN's), home health aides (HHA's), specialized nursing services such as infusion therapy, social workers, and physical, speech, rehabilitational and occupational therapies. BayCare HomeCare accepts Medicare, Managed Care, Private Pay, and Medicaid.* Nurses maintain contact with physicians and other caregivers when necessary to ensure proper coordination of care.

BayCare HomeCare offers a range of structured clinical programs for the management of chronic and post surgical conditions in the comfort of the patient's home. Clinical teams use nationally accepted best practice standards to teach patients, family members, and other caregivers how best to manage the disease process. This state of the art disease management process prevents hospital readmissions and helps patients maintain optimal health in the home. Our disease management programs include:

Private Duty Care
BayCare HomeCare offers a self-pay option for unfunded nursing care that does not require a physician's order. Our well organized Private Duty Program can be considered when patients and their families would feel more comfortable with a little bit more supervision. The BayCare HomeCare Private Duty Program works closely with the discharge planning organizations located at the hospitals within the BayCare Health System. These affiliations include Morton Plant Mease Primary Care and Healthpoint Medical Services.
The telemonitoring program provides remote nurse management of a patient's vital signs to monitor their condition, providing immediate support for management of changes in their condition that may warrant intervention. Patient's physicians are provided with statistics through daily communication if necessary. Telehealth monitoring may be implemented with heart disease and/or other conditions.
A new technology that eliminates unnecessary catheterizations, reduces incidences of urinary tract infections (UTI), minimizes antibiotic use associated with UTI, prevents undiagnosed urinary retention and minimizes catheter-related damage to upper urinary tract.
