Adult Comprehensive Community Service Team (CCST)

Program Description

CCST clusters the traditional services of targeted case management, supported employment, supported living, information and referral, outpatient, intervention and aftercare to work with identified individuals and their family and significant others involved in the recovery.

The ultimate goal of CCST is to re-establish normal roles for the identified individual within their community, as well as to develop their personal supports networks and increase their quality of life. The program helps implement the concept of recovery with clinical, financial and consumer information in order to support new management decision-making functions for the development of recovery-oriented service strategies. In addition, our recovery-oriented, service strategies reach beyond the critical issues of assuring personal safety and managing symptoms and focus on the rebuilding of full, productive lives for individuals.

Target populations are adults with mental illness who require assistance in identifying goals and making choices to promote and facilitate recovery, including individuals with mental health problems or severe and persistent mental illness. Enhanced services for special populations are provided for adults with co-occurring disorders, adults with borderline personality disorders, adults at risk for residential psychiatric placement and adults with forensic involvement.

Hours:

Monday-Thursday, 8 a.m. to 7 p.m.; Friday, 8 a.m. to 5 p.m.; Weekends as scheduled.
